Initial Comments:
An off-site follow up survey conducted on February 2, 2017, found that Ark of Life Home Health, Llc. had corrected the deficiencies cited under the requirements of 28 Pa. Code, Part IV, Health Facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of a state re-licensure survey conducted on December 9, 2016.
